Anticoagulation is the cornerstone of stroke prevention in atrial fibrillation, venous thromboembolism, mechanical heart valves, and selected hypercoagulable states. The pharmacology has evolved dramatically with the introduction of direct oral anticoagulants (DOACs) — apixaban, rivaroxaban, edoxaban, and dabigatran — that have largely replaced warfarin for most non-valvular indications. The clinical stroke neurologist must navigate DOAC selection, dose adjustment for renal function, peri-procedural management, reversal in major bleeding, and the special considerations in cardiac thrombus, antiphospholipid syndrome, and mechanical valves. This page covers the modern anticoagulation landscape.

Coagulation Pharmacology Targets

  • Vitamin K-dependent factors: II (thrombin), VII, IX, X, proteins C and S; targeted by warfarin.
  • Thrombin (factor IIa): targeted by dabigatran (direct), heparins (via antithrombin).
  • Factor Xa: targeted by apixaban, rivaroxaban, edoxaban (direct); LMWH (via antithrombin).
  • Antithrombin amplification: heparins, fondaparinux.

Direct Oral Anticoagulants (DOACs)

Apixaban (Eliquis)

  • Mechanism: direct factor Xa inhibitor.
  • Dose: 5 mg BID (or 2.5 mg BID if ≥2 of: age ≥80, weight ≤60 kg, Cr ≥1.5).
  • PK: half-life 12 hours; CYP3A4 + P-gp metabolism; partial renal clearance.
  • Advantages: lowest GI bleeding among DOACs; safe in moderate CKD; even in some severe CKD/dialysis (data emerging).
  • Pivotal trial: ARISTOTLE (vs warfarin in AF).

Rivaroxaban (Xarelto)

  • Mechanism: direct factor Xa inhibitor.
  • Dose: 20 mg daily (or 15 mg if CrCl 15-50; taken with food).
  • PK: half-life 5-13 hours; CYP3A4 + P-gp; partial renal clearance.
  • Drug interactions: avoid strong CYP3A4 inducers (rifampin, carbamazepine, phenytoin); cautious with strong inhibitors.
  • Pivotal trial: ROCKET-AF.

Edoxaban (Savaysa)

  • Mechanism: direct factor Xa inhibitor.
  • Dose: 60 mg daily (or 30 mg if CrCl 15-50; 30 mg if body weight ≤60 kg or concurrent strong P-gp inhibitor).
  • NOT recommended if CrCl >95 (paradoxically reduced efficacy in AF).
  • Pivotal trial: ENGAGE-AF-TIMI 48.

Dabigatran (Pradaxa)

  • Mechanism: direct thrombin inhibitor.
  • Dose: 150 mg BID (or 75 mg BID if CrCl 15-30).
  • PK: half-life 12-17 hours; 80% renal clearance.
  • Dyspepsia common: ~10-20% of patients.
  • Pivotal trial: RE-LY.
  • Reversal: idarucizumab (Praxbind) — specific monoclonal Fab; FDA-approved.

Warfarin

  • Mechanism: blocks vitamin K epoxide reductase → depletes vitamin K-dependent factors (II, VII, IX, X, proteins C, S).
  • PK: CYP2C9 (primary); CYP1A2, CYP3A4.
  • Pharmacogenomics: VKORC1 polymorphism affects warfarin sensitivity; CYP2C9 polymorphism affects clearance.
  • Target INR: 2.0-3.0 for AF; 2.5-3.5 for mechanical mitral valves.
  • Drug interactions: many — amiodarone, sulfamethoxazole, fluconazole, metronidazole increase INR; vitamin K dietary changes affect INR.
  • Indications still preferred over DOAC: mechanical heart valves, valvular AF (moderate-severe mitral stenosis or rheumatic), severe renal impairment with anticoagulation needed.
  • Reversal: vitamin K (oral or IV); prothrombin complex concentrate (PCC, 4-factor); fresh frozen plasma (less preferred).

Heparins

Unfractionated Heparin (UFH)

  • Mechanism: binds antithrombin → accelerates inhibition of thrombin and factor Xa.
  • PK: IV; rapid onset; half-life 60-90 min; monitored by aPTT or anti-Xa.
  • Uses: acute stroke (rarely indicated for routine ischemic stroke — IS-AF, IST trials), cardiac thrombus bridging, CVT, large-vessel disease, dissection (controversial).
  • Heparin-induced thrombocytopenia (HIT): type II is immune-mediated, paradoxical thrombosis; switch to argatroban or bivalirudin.
  • Reversal: protamine sulfate.

Low-Molecular-Weight Heparin (LMWH)

  • Enoxaparin, dalteparin, tinzaparin: more selective for factor Xa than thrombin.
  • SC; not routinely monitored.
  • Dose by weight; renal adjustment.
  • Uses: DVT/PE treatment, prophylaxis, bridging.
  • Less HIT than UFH.
  • Partial reversal by protamine.

Fondaparinux

  • Synthetic pentasaccharide; pure factor Xa inhibitor via antithrombin.
  • SC; renally cleared.
  • Alternative for HIT history.
  • No specific reversal.

Indications and Selection

Atrial Fibrillation

  • Score: CHA₂DS₂-VASc.
  • Score ≥2 in men, ≥3 in women: anticoagulation recommended.
  • Score 1 in men, 2 in women: consider anticoagulation.
  • Score 0: no anticoagulation.
  • DOACs preferred over warfarin for non-valvular AF.
  • Selection by patient factors: bleeding risk (HAS-BLED), renal function, drug interactions, dosing convenience, cost.

Cardioembolic Stroke from AF

  • Timing of anticoagulation after stroke: traditional 7-14 day delay; emerging data (TIMING, ELAN) suggest earlier may be safer for many.
  • Larger strokes, hemorrhagic transformation: delay longer.
  • TIA: anticoagulation can often start within 1-2 days.

Cardioembolic Stroke from Other Sources

  • Mechanical heart valve: warfarin (DOACs contraindicated).
  • Cardiomyopathy with LV thrombus: warfarin or DOAC (data emerging).
  • Recent MI with anterior wall akinesis: anticoagulation for ~3 months.

Cerebral Venous Sinus Thrombosis

  • Acute: heparin (UFH or LMWH).
  • Long-term: warfarin or DOAC (TRUST-CVT, RE-SPECT CVT supportive of DOAC).
  • Duration: 3-12 months depending on cause.

Carotid/Vertebral Dissection

  • Antiplatelet vs anticoagulation: CADISS trial showed equivalence.
  • Either acceptable; antiplatelet often easier.
  • Duration 3-6 months.

Antiphospholipid Syndrome

  • Warfarin: traditional first-line.
  • DOACs: failed in TRAPS, ASTRO-APS trials — INFERIOR to warfarin in high-risk APS (triple positive).
  • DOACs may be acceptable in single-positive APS without prior thrombosis.

Reversal Agents

  • Idarucizumab (Praxbind): dabigatran reversal; FDA-approved.
  • Andexanet alfa (Andexxa): factor Xa inhibitor reversal (apixaban, rivaroxaban); FDA-approved.
  • 4-factor PCC (Kcentra): warfarin reversal; off-label for DOACs (anti-Xa).
  • Vitamin K: warfarin reversal (slow, hours; only for non-emergency).
  • Protamine sulfate: UFH reversal; partial for LMWH.
  • FFP: warfarin reversal; less preferred than PCC.
  • TXA, DDAVP: adjunctive in bleeding.

Peri-procedural Anticoagulation Management

  • DOACs typically held 24-48 hours pre-procedure (longer if renal impairment).
  • Bridge UFH/LMWH not needed for most DOAC patients (rapid onset/offset).
  • Warfarin: 5 days pre-procedure; bridge LMWH if high thromboembolism risk.
  • Restart timing depends on procedure bleeding risk and patient factors.

Anticoagulation in Special Populations

Pregnancy

  • Warfarin: teratogenic (especially weeks 6-12 — “warfarin embryopathy”); CNS abnormalities later.
  • DOACs: contraindicated.
  • LMWH: preferred; daily SC; bleeding management around delivery.
  • UFH: alternative if cost concern.

Cancer-Associated Thrombosis

  • Historically: LMWH preferred.
  • Now: apixaban, rivaroxaban, edoxaban acceptable (Hokusai-VTE, AVERT data).
  • Higher GI bleeding in some cancers (luminal GI cancer): consider apixaban or LMWH.

Renal Impairment

  • Apixaban: safest DOAC in severe CKD; some use in dialysis.
  • Rivaroxaban, edoxaban: avoid if CrCl <15.
  • Dabigatran: contraindicated in CrCl <15.
  • Warfarin: usable in any renal function.

The timing of anticoagulation initiation after acute cardioembolic stroke has been one of the most clinically important and previously underanswered questions in stroke neurology. The traditional “1, 3, 6, 12” rule — initiate within 1 day for TIA, 3 days for small stroke, 6 days for moderate, 12 days for large stroke — was based on observational data and expert consensus. The recent TIMING (2022) and ELAN (2023) randomized trials have transformed this paradigm: earlier initiation appears safer than previously thought for most patients. ELAN, in particular, showed that initiating DOAC within 48 hours for minor/moderate stroke or within 6-7 days for severe stroke was non-inferior to delayed initiation, with no increase in symptomatic intracranial hemorrhage and a trend toward reduced recurrent stroke. The clinical implication is that guidelines are evolving toward earlier anticoagulation, particularly with DOACs that have favorable pharmacology (rapid onset/offset). Practical adjustments: TIA — start within 24 hours; small ischemic stroke (NIHSS <10, no hemorrhage) — within 1-3 days; moderate (NIHSS 10-20) — within 3-7 days; severe (NIHSS >20 or hemorrhage) — 7-14 days. The lesson generalizes: evidence-based practice continues to evolve based on randomized data, and clinical neurologists must stay current with trial results that can dramatically shift therapy timing decisions. For AF management, the “wait and see” approach is being replaced by data-driven early initiation in suitable patients.

Pitfalls and Pearls

  • DOACs preferred over warfarin in non-valvular AF: less ICH, better adherence.
  • Apixaban: factor Xa; lowest GI bleeding; safest in severe CKD; BID.
  • Rivaroxaban: factor Xa; once daily with food; CYP3A4 inducer interactions.
  • Edoxaban: factor Xa; avoid if CrCl >95 (paradox).
  • Dabigatran: direct thrombin; 80% renal; dyspepsia; idarucizumab reversal.
  • Warfarin preferred: mechanical valves, valvular AF (rheumatic MS), APS triple positive.
  • CHA₂DS₂-VASc: AF stroke risk stratification; anticoagulate if ≥2 (men) or ≥3 (women).
  • CADISS: dissection antiplatelet ≈ anticoagulation.
  • APS: warfarin preferred over DOAC in triple-positive APS.
  • Timing post-stroke: ELAN/TIMING — earlier is safer than previously thought.
